OUR SERVICES
West of Hartford@Home will provide the following services, all
accessible with one phone call:
- Referrals to vetted vendors who do good work at reasonable prices
(in some cases at a reduced rate) and who are reliable and trustworthy
- Transportation to grocery shopping and medically necessary
appointments
- Help with computers, tax preparation, checkbook balancing and bill
paying
- Access to social and cultural events going on in the community, as
well as group rates at local restaurants
- Member interest groups such as book clubs, discussion groups, art
classes
- Shared activities that help maintain old friendships and create new
ones
- Reassurance to family and friends
- Referrals to health services, possibly with group discounts, and help
in dealing with insurance companies, Medicare and Medicaid
- Trained volunteer patient advocates to assist members with medical
visits
- Referral to reliable health home care providers
- Weekly contact with fulltime director for each member to insure
his/her needs are being met and to get new ideas for services to
provide.
